Title: Housing Specialist Division: APAIT
FLSA: Non-Exempt Supervisor: Associate Director
Pay Range: $15.38/hr. – $16.82/hr. (DOE), plus comprehensive benefits

Summary
Under the supervision of the Associate Director, the Housing Specialist will provide housing support services to people living with HIV/AIDS. Housing support services will include researching affordable housing resources, providing referrals and resources, coordinating referrals, maintaining case notes and conducting follow-up; performing related administrative tasks; and managing local accordable housing resources information. This is a full-time position requiring flexible hours including evenings, weekends, and some holidays.
Essential Functions

  1. Assist clients with applications for housing related services, short-term housing assistance, and permanent housing placement.
  2. Provide information and referrals regarding program and services available to people living with HIV/AIDS.
  3. Determine eligibility, calculate income allowances, assets, and rent and perform data entry and/or continued eligibility.
  4. Develop relationships with other community agencies in order to facilitate an effective referral system.
  5. Remain familiar with current U.S. Department of Housing and Urban Development federal regulations as related to Section 8.
  6. Work with client case managers and other care providers to ensure appropriate care coordination.
  7. Facilitate and mediate communications with client and landlord needs.
  8. Ability to work overtime and perform duties during weekends, holiday, and evenings when required.
  9. Regular attendance.

Minimum Qualifications – Knowledge, Skills and Abilities Required

  1. 1. BA/BS in social work, human services, or equivalent or two years of similar work experience.
  2. 2. Knowledge and demonstrated experience in working with HIV/AIDS population, impoverished and under-served communities and groups.
  3. 3. Ability to work with diverse communities including but not limited to people of color, gay/lesbian/bisexual/ transgender/questioning communities, monolingual and/or undocumented immigrants, persons living with HIV/AIDS, substance users, and homeless persons, of varying ages.
  4. 4. Strong interpersonal skills and ability to relate to staff from a variety of cultures, languages and educational backgrounds.
  5. 5. Demonstrated experience and knowledge of PC hardware and software (i.e.,Windows, DOS, word processing, spreadsheets and databases).
  6. 6. Valid California driver’s license, plus proof of car insurance.
  7. 7. Reliable transportation.
  8. 8. Verification of employment eligibility.
  9. 9. Passing of background check.
  10. 10. Updated tuberculosis test (annual requirement) – read, reviewed and dated by a clinician.

Non-Essential Qualifications – Knowledge, Skills and Abilities
1. Fluent in Spanish and/or an Asian language.

Title: Outreach and Engagement Specialist Division: APAIT

Summary
Under the supervision of the Division Director, a strong team player and people-oriented Outreach and Engagement Specialist (O&E Specialist) will coordinate and implement strategic outreach and community engagement efforts to increase awareness of APAIT’s outpatient substance abuse treatment program for women of color in LA County. The candidate must have the ability to build and maintain rapport with community members, community planning groups, and stakeholders. Experience working with diverse ethnic and cultural communities is a must, including, but not limited to the following populations: people of color, gay/lesbian/ bisexual/ transgender/questioning community, monolingual and/or undocumented immigrants, substance users, and homeless persons, of varying ages. This is a full-time position requiring flexible hours including evenings, weekends, and some holidays.
Essential Functions
1. Conduct outreach activities to promote services and recruit program participants.
2. Establish and maintain partnerships with Los Angeles based substance abuse treatment programs to identify and recruit potential clients.
3. Implement outreach and recruitment activities targeting cisgender women of color throughout LA County.
4. Conduct client follow-up activities to ensure that a minimum 33% of referred individuals are linked to APAIT’s outpatient substance abuse treatment program.
5. Represent and advocate on behalf of the agency and the community at related collaborations, community planning groups, social organizations, conferences, and other events as assigned.
6. Other projects as assigned to meet program and agency mission, goals and objectives.
7. Ability to work overtime and perform duties during weekends, holiday, and evenings when required.
8. Regular attendance.
Minimum Qualifications – Knowledge, Skills and Abilities Required
1. High school diploma or GED equivalent plus a minimum of two years’ experience in substance abuse and/or HIV/AIDS work.
2. Established relationships with Los Angeles based substance abuse treatment programs and/or facilities.
3. Ability to work with diverse communities including but not limited to multi-gender, ethnic, generation, lingual, dual or multi-diagnosed, etc.
4. Great attitude and people skills a must.
5. Must be fluent in Spanish (read, speak, and write).
6. Ability to communicate effectively both verbally and in writing with strong leadership skills.
7. Good problem solving and conflict resolution skills.
8. Reliable transportation.
9. Valid California driver’s license, plus proof of current car insurance.
10. Verification of employment eligibility.
11. Passing of background check.
12. Updated tuberculosis test (annual requirement) – read, reviewed and dated by a clinician.

Salary: $20.00/hr. – $23.00/hr. (DOE), plus comprehensive benefits

Summary

Under the supervision of the Community Health Education and Support Services (CHESS) Program Manager, the LTC Specialist will identify and link HIV-positive clients to HIV medical care through the implementation of outreach, brief intervention sessions, graduated disengagement, and follow up. Experience working with diverse ethnic and cultural communities is a must, including, but not limited to the following populations: people of color, gay/lesbian/ bisexual/ transgender/questioning community, monolingual and/or undocumented immigrants, substance users, and homeless persons, of varying ages. This is a full-time position and will require flexible hours including evenings, weekends, and some holidays.

Essential Functions

  1. Identify and link HIV-positive clients to HIV medical care through the implementation of outreach and brief intervention sessions per agency and Division of HIV and STD Programs (DHSP) guidelines.
  2. Conduct patient follow-up to ensure services were obtained and provide additional support as needed.
  3. Implement programmatic scope of work at a minimum of 85% completion rate.
  4. Report on intervention activities and propose strategies to address challenges in implementation and emerging trends.
  5. Provide necessary programmatic documentation, including the timely completion of reports, administration of evaluation tools, and other program related assignments.
  6. Assist in the coordination and participate in community events, health fairs, and festivals.
  7. Represent the agency by attending local planning groups, community forums, and collaborations or consortium meetings.
  8. Other projects as assigned to meet program and agency mission, goals and objectives.

Minimum QualificationsKnowledge, Skills and Abilities Required

  1. Master’s degree in Public Health, Sociology, Psychology, Social Services, or other Public Health related field and minimum of one year experience in providing direct services to individuals living with HIV/AIDS.
  2. Knowledge of HIV/AIDS risk behaviors, STDs, human sexuality, substance addiction, and HIV behavior change principles and strategies.
  3. Ability to develop and maintain written documentation (assessments, service plans, progress notes, monthly reports, and other programmatic documentation).
  4. Reliable transportation.
  5. Valid California driver’s license, plus proof of current automobile insurance.
  6. Verification of employment eligibility.
  7. Passing of background check.
  8. Updated tuberculosis test (annual requirement) read, reviewed, and dated by a clinician within 6 months of start date.
  9. Great attitude and people skills; strong interpersonal skills and ability to relate to individuals from diverse backgrounds.
  10. Ability to coordinate multiple projects and tasks simultaneously in a high-pressure work environment.
  11. Ability to master a wide-range of information, adapts, take initiative, and work in a team-centered environment.
  12. Experience and knowledge MS Office (Word and Excel a must)
